May 10, 2022 – The Cook County Aging Coalition is pleased to introduce Aging Well Resources, a new “one-stop” guide to local programs, events, and support. Aging Well Resources includes a comprehensive easy-to-use website designed for residents of Cook County, as well as a Care Navigator for phone assistance. It’s never been easier for Cook County seniors to find the information they need! For those who would rather speak directly to someone or who need help understanding support options, Care Navigator Christie John offers friendly assistance by phone by calling (218) 231-8903 or toll-free (855) 444-3910.
AgingWellResources.org provides access to local programs, regional resources, and a calendar of events for seniors. There are simple links to request more information about a program, find agency websites and phone numbers, or print brochures. The website’s straightforward design and uncomplicated navigation are the work of Grand Marais-based Molly Larson of Sweetwater Design.
“Aging Well Resources will also make it easier for our aging service partners to assist their clients. They will be able to depend on us to send referrals and update the website with current resource and event information”, said Christie John.
Aging Well Resources is a collaborative effort of the Cook County Aging Coalition, which is comprised of representatives from many area service providers, including Care Partners of Cook County, Cook County Council on Aging/The Hub, Cook County Public Health & Human Services and In-Home Support, Grand Portage Health Service, Grand Portage Human Services, Grand Portage Elderly Nutrition Program, North Shore Health Care Foundation, and North Shore Health. Care Partners of Cook County was awarded a $65,000 grant through Northland Foundation’s Integrated Rural Community Aging Program to help the Coalition improve access to aging services.

About the Northland Foundation’s Integrated Rural Community Aging Program
Care Partners of Cook County and its coalition members are one of eight sites in northeastern Minnesota selected to participate in the Community Planning and Care Coordination Project, one facet of the Northland Foundation’s new, three-year, $3.75 million Integrated Rural Community Aging Program. Northland and its key program partner, the Arrowhead Area Agency on Aging, will provide resources and assistance over the next two and a half years to the help fund exciting projects among the eight coalitions in the region to create well-connected programs and services that support rural older adults and their family caregivers.
The Community Planning and Care Coordination Project is just one strategy of the Integrated Rural Community Aging Program.
Northland will also leverage its nationally recognized AGE to age program in 18 sites across the region to increase social engagement among older adults while helping children and youth thrive by working with Tribal Nation partners to provide resources that will help them further support their Elders; and supporting workforce strategies to increase the quantity and quality of caregivers for older adults.
