Our Vision
A healthy community for all in Cook County
The North Shore Health Care Foundation is an independent nonprofit 501(c)(3) organization that proactively identifies opportunities to expand equitable health care and healthy living in Cook County. We champion innovative solutions that have a direct impact on emergent issues affecting our community. Grounded in the unique landscape of Northeastern Minnesota, we strive to weave healing relationships and meet the needs of all residents.
The Foundation awarded more than $1.6 million in grants over thirty years, and now acts as a collaborative convener, investor, and developer of incubated programs. We have created a portfolio of successful and award-winning programs across the lifespan that address critical gaps in health services, including Early Childhood & Family Services, The Oral Health Task Force, Restorative Justice & Truancy Programs, Emergency Workforce Solutions, and Care Partners. We know that supporting healthy families builds a strong community. More than thirty years ago, our founders began building healthy solutions–we hope you will join us in building healthy communities today!

Listings That Give Back: Foundation Home Real Estate Program
The Foundation Home Program is an innovative partnership with local real estate professionals in which a percentage of the commission from select property listings is donated to the Foundation to invest in local health and emergency services.
